Should Dentists and Family Doctors Collaborate on Their Patients’ Overall Health?

A recent report has outlined a range of benefits that could result from improved collaboration and communication from dentists and family doctors. The report shows that a link between oral health and physical health conditions exist. Moreover, it points out that this integration could result in improved treatment outcomes and prevention of many conditions.

As part of our wider healthcare, we typically see a family doctor for issues relating to our physical health, while speaking to our dentist for our oral health needs.

While this is perfectly acceptable, a recent report has urged for there to be more collaboration between dentists and family doctors in order to maximize the effectiveness of the services provided [1].

By working closely, doctors, dentists and patients can all benefit, especially with there being a strong connection between oral health conditions and physical health.

The Consensus Report

This conclusion came from a report issued by the World Organization of Family Doctors (WOFD) [1]. Subsequently, they published their findings in the Journal of Clinical Periodontology [1].

This was a form of report called a “Consensus Report”. This is where a selection of experts come together to present a finding that they all agree on. This particular consensus report involved 18 global leading experts in periodontology and family medicine [1].

The report highlighted the association between periodontal disease and physical health conditions like cardiovascular disease, diabetes and respiratory diseases [1].

The authors have suggested that family doctors and oral health professionals should exchange information and refer patients to one another [1]. This could help to treat these linking conditions, promoting wider benefits to health.

The authors are preparing an outreach campaign based on the findings of this report. They aim to canvas policymakers, as well as those on the frontline – dentists and family doctors [2].

The report provides further evidence that shows gum disease is independently associated with cardiovascular diseases, diabetes and respiratory diseases [1]. Furthermore, this appears to include an enhanced chance of Covid-19 complications [1].

What can collaboration achieve?

The report points out that there are several benefits to closer collaboration. These include effective prevention, early detection and management of widespread systemic health conditions [1].

The report used an example that involved dentists making family doctors aware if a patient of theirs is going through the early phase of gum disease [1]. This would have a negative effect on the health of the patient and increase their chances of getting a physical health condition outlined above. Therefore, by knowing this knowledge early, the family doctor could try to implement strategies to protect the patient from such conditions.

Additionally, another example given was that oral health professionals – when treating patients with periodontitis – should let them know that their risk of cardiovascular diseases is higher [1]. They could then report this to their family doctor.

This research builds on previous findings by the WOFD [2]. In the past, they had found a link between gum disease and cardiovascular diseases [2]. But this report goes further and will prove very helpful.

President of the WOFD, Shlomo Vinker, commented that “the current split between dental health professionals on one side, and systemic disease professionals on the other makes no sense” [2]. Therefore, he has urged there to be “greater integration and better sharing of information” [2].

The authors commented on their findings. Lead author David Herrera said that “our paper presents a critical update of the evidence supporting the associations between periodontitis and very important systemic conditions” [2].

What Have Dentists Said About Dental Funding in the New Federal Budget?

The Federal Government have announced their new budget, with the announcement of an increase in funding for dental care resulting in widespread joy from oral health professionals. The funding will increase to $13billion over the next five years. Moreover, this will form part of the new national dental care plan. Further details about this plan have also been released.

Dentists have applauded the Federal Government’s decision to increase funding for dental care as part of their 2023 Federal budget.

The government have committed to increase funding for the National Dental Care Plan to $13billion over the next five years, beginning in the 2023-24 year [1].

This is another example of how the government is contributing to dental care in Canada, following the creation of the temporary national dental benefit at the end of 2022.

Dental care in Canada

Compared to many countries, the standard of dental care in Canada is strong. But with many requiring treatment each year, further work is needed to improve oral health.

Almost two-thirds of Canadians have a form of dental insurance [2]. But those that don’t face large hospital bills, A&E visits and intense dental pain.

However, the national dental benefit which was launched at the end of 2022 sought to provide more access to dental care for children from families with a low-middle income.

This is just a temporary measure though, with a wider and more comprehensive plan due to be in place by 2025. This new plan will offer support to a wider range of people, rather than just children.

Given the large demand for dental services in Canada and rising population numbers, having an increase in funding was deemed to be crucial by some politicians.

The increase in funding

The government have announced their 2023 Federal budget [1]. As part of this, they have provided more details on how the eventual national dental care plan will look.

The projected cost is much higher than the ruling Liberal Party first estimated. The final program is now due to cost $13billion over five years [1]. This is a big increase on the $5billion they had originally earmarked for the program. The aforementioned temporary benefit will be phased out by June 2024 [3].

This year, eligibility for the benefit will extend to those under the age of 18, seniors and people with disabilities [3]. Those hoping to access the benefit will still need to meet the low-middle income criteria and not have insurance.

Finance Minister Chrystia Freeland said that “by the end of 2023, we will begin rolling out a dental-care plan for what will eventually be up to nine million uninsured Canadians” [3].

The update was praised by leader of the New Democratic Party – Jagmeet Singh – who has been a long-term advocate for improved access to dental care in Canada. This was one of the most important aspects of the Liberal/NDP confidence-and-supply deal.

Singh said that “I think Canadians are going to be happy” with this announcement [3]. He also commended his party’s work on the issue, saying that “we forced the government to do this” [3].

However, politicians in the Conservative Party largely criticized the budget increase. Leader Pierre Poilievre condemned the NDP, saying that they are “bankrupting households” and “forcing seniors to choose between heating and eating” [3].

Amidst criticism, Freeland admitted that the plan “does cost a lot of money”, but said that the increased cost was a “necessary expansion of healthcare” [3].

How have dentists reacted?

Understandably, dentists were delighted with the news. Dr. Lynn Tompkins – President of the Canadian Dental Association – praised the “historic investment” [1]. She suggested that the budget highlighted the “government’s continued commitment” towards improving the oral health of Canadians [1].

Moreover, Tompkins said that the “CDA has long advocated for targeted investments to improve access to oral health care” – with access being a key issue [1]. Furthermore, Tompkins said that this new budget will “yield even more long-term gains in oral and overall health” [1].

Meanwhile, Dr. David Lim of the British Columbia Dental Association said that his organization is “encouraged that the federal government recognizes the link between oral health and overall health” [1]. The topic of the link between oral and overall health is very important. Hopefully, the increased budget will mean this link is known by more people.

In other news that has pleased dentists, the Liberals have suggested that $23million should be put aside over the next two years to collect better data on oral health, which could help to make better informed decisions when implementing the dental plan.

Overall, this is very positive news, with the hope now that oral health across Canada will strengthen in the years to come. This should result in strong health improvements too across the nation for future generations.

In line with World Oral Health Day 2023, the World Health Organization has made some recommendations that aim to improve the oral health of people all around the world. Their main focus is on the South-East Asia region – owing to their oral health problems. However, the oral health of everyone is crucial, with recommendations also targeted at governments around the world.

World Oral Health Day 2023 is upon us, with this day being an annual reminder of the importance of oral health and its impact on our overall health.

The World Health Organization (WHO) have marked the occasion by making a series of recommendations [1]. They continue to advocate for improved dental care around the world.

What is World Oral Health Day?

World Oral Health Day takes place once a year, and celebrates positive oral health whilst raising awareness in the importance of looking after our teeth.

The day is organized by the World Dental Federation. Owing to substantial research carried out in recent years, the awareness of the importance of strong oral health within the scientific community is at an all-time high. Now, scientists are working out how to transfer this awareness to the general public.

The importance of World Oral Health Day

The importance of World Oral Health Day cannot be underestimated. The WHO estimate that approximately 3.5billion people around the world are affected by oral diseases [1].

Moreover, the WHO have also stated that between 1990 and 2019, estimated numbers of people that suffered from oral diseases increased by more than 1billion people – which exceeded population increase during this period [1].

These findings are deeply worrying, and show how oral health problems are affecting so many people. Therefore, the WHO have now made some recommendations that aim to assist the human population with their oral health.

Recommendations from the WHO: South-East Asia

The WHO’s main focus is on the South-East Asia region – which is a region where oral health is typically poor [2]. These nations include Cambodia, Brunei and Vietnam among others.

One of the main recommendations from the WHO is for countries in the South-East Asia region to accelerate implementation of the Action Plan for Oral Health 2022-2030 – an 8-year plan that sets out a plan for general improvement in oral health across the region [1].

The South-East Asia region is of particular importance. In 2019, the WHO found that the region had an estimated 900million cases of untreated dental caries, gum disease and tooth loss [1]. Moreover, the area also has a high number of oral cancer cases. Therefore, this area needs attention.

The WHO has been providing support to all nations in the region since 2014. They have been making steady progress. 10 nations in the region are now offering oral health screening in primary health care – with the aim of detecting oral problems at an early stage [1].

Moreover, 6 nations have improved their treatment options for emergency dental problems [1]. A further 5 nations have imposed taxes on sugary drinks. All nations in the region are seeing improvements in terms of lessening tobacco use.

This action plan forms a wider goal of the WHO, which concerns achieving universal coverage for oral health by 2030 [1]. While this might be seen as ambitious, it is achievable, and work is being done to assist in this.

General Recommendations

While the main focus is on South-East Asia, the WHO has also put forward general recommendations. This includes providing adequate financing for dental programs. Additionally, this involves ministries of health providing “dedicated, qualified, functional, well-resourced and accountable oral health units that are integrated with existing internal structures and collaborate across programme areas” [1].

Another recommendation concerns promoting the importance of oral health from a young age, typically through education in school. The WHO note that education should also involve reducing risk factors like tobacco use and high sugar diets [1].

They also suggest that countries should establish systems to collect detailed information on oral health services [1]. This can help oral health professionals to notice patterns and apportion funding to areas that require the most attention. Ahead of the upcoming federal budget announcement, the New Democratic Party have urged the Liberal Party to invest in expanding dental coverage. This would include helping teenagers, seniors and those living with a disability. Increased investment would have a very positive impact on many people.

The New Democratic Party (NDP) have urged the ruling Liberal Party to invest in expanding dental coverage to teenagers, seniors and those living with a disability [1].

The request comes as part of the confidence and supply agreement between the NDP and Liberal Party.

This agreement provides the NDP with leverage, and it will be interesting to see how the Liberals react, with the next federal budget being released soon.

Dental care in Canada: The current situation

Currently, many Canadians have dental coverage. But not everyone has this – meaning they are often deprived of crucial dental care.

The recent National Dental Benefit is helping children from low-middle income families to have more access to dental services. While it is due to be extended to adults in the future, for now it has a limited number of beneficiaries.

When asked about this, Singh said “we want to see commitment to the dollars necessary for it to roll out to that national program” [1].

Furthermore, Singh has said that there are “some signals Liberals were open to it”, but that the NDP must “continue to apply pressure to make it happen” [1].

Singh appears to believe that such changes are realistic, suggesting he is “confident” that the Liberals will follow through with his requests [1].

Meanwhile, the Conservatives – who have traditionally opposed changes to the dental care system – have requested different changes to the budget, including lower taxes [1].

The potential impact

If there was increased funding for dental care in Canada, it would provide significant help for many people around the country. As mentioned earlier, many people face barriers to receiving dental care.

By not having regular check-ups, a person runs the risk of missing out on crucial treatment. Inevitably, their oral health will worsen, leaving them in excruciating pain and discomfort.

Therefore, by having increased funding, more people will have access to dental care. This would hopefully result in improved oral health for Canadians, and lessen the burden placed on healthcare establishments.

Overall, there are many positives to this request. But as the NDP are not the ruling party, there is no guarantee that their request will be sanctioned. We will need to wait and see how the federal budget looks when it is released soon.

Does a Lack of Access to Fluoridated Water Affect Children’s Oral Health?

Further research into the topic of Water Fluoridation has recently taken place. The research sought to find out if a lack of access to fluoridated water impacted oral health in children. When compared with children that did have access to fluoridated water, there were concerning findings.

Water Fluoridation is often considered a controversial subject, but research has consistently shown that its impact on children’s oral health is very positive.

The results of further research into the topic has recently been released, which looked at the oral health of children that have no access to fluoridated water [1].

Background

Water fluoridation refers to the action of adding a small concentration of Fluoride to the public water supply [2]. This is done as a means of protection from cavities, with research showing a strong connection between fluoridated water and less tooth decay [2].

Fluoride is a key ingredient in toothpaste. Therefore, by adding it in small concentrations to the public water supply, the intention is to assist in protecting teeth.

However, water fluoridation can be a controversial topic. For example, its use is banned in many countries, mainly due to concerns of potential over-exposure to fluoride, the ethics behind adding something to the public water supply and the lack of choice given to citizens [3].

Water fluoridation was first introduced in Australia in 1953 [4]. Given the size of Australia, not all parts of the nation have access to fluoridated water.

The Research

A team of researchers from La Trobe University carried out a new study. Subsequently, the findings were published in the Australian Journal of Rural Health [1].

The research involved looking at the state of Victoria – a vast state located in south-eastern Australia. They found that one-third of rural towns in the state that have populations of over 1,000 do not have access to fluoridated water [1].

The study involved looking at the oral health profiles of rural towns that did not have water fluoridation. Indicators like preventable hospitalisations due to dental conditions, tooth decay levels and filled or missing teeth were all noted [1].

Then, these levels were then compared with the state average of Victoria – whose large cities and towns nearly all have access to fluoridated water. The idea was to then compare the figures.

The results showed a clear disparity between the two data sets – with the authors noting a “significant” difference between non-fluoridated and fluoridated areas [1].

Overall, the main finding was that those in non-fluoridated areas had much higher than average preventable hospital admissions linked to oral health in children aged 0-9 years [1].

Additionally, more than 50% of children aged 0-12 living in these non-fluoridated towns had above average rates of decayed, missing and filled teeth. This percentage rose to 78% for children aged 0-5 [1].

This clearly shows that water fluoridation has a big impact. The above statistics show that children of all ages in non-fluoridated areas immediately have disadvantages in terms of their oral health in comparison to children in fluoridated areas.

The researchers found that this lack of water fluoridation meant that almost 150,000 residents across 203 towns were having their teeth impacted. Furthermore, children were impacted the most [1].

Author comments

The authors were concerned at these findings. Lead author Dr. Virginia Dickson-Swift discussed the findings. She urged officials to take action, saying that water fluoridation is “supported by overwhelming scientific evidence” [4].

Continuing, Dickson-Swift said that water fluoridation is “internationally recognized as one of the most successful public health interventions over the past 20 years, supported by groups like the World Health Organization and the Australian Dental Association” [4].

This combination is proving very popular, and provides many benefits to all parties. For patients, they get free dental care, while dentists get crucial training, and the chance to specialize in certain areas.

The University of Alberta are helping many people all in one go with a specialized program – with dentists being trained whilst providing free dental care to patients.

The program is ran by the University of Alberta’s School of Dentistry. Their conscious IV sedation program runs five times a year, and provides the general public with free check-ups and simple fillings [1].

The program

As mentioned, this is a specialized program ran by the School of Dentistry at the University of Alberta. It is an entirely unique program.

The program gives licensed practicing dentists the chance to attain certification or specialization requirements [1]. For example, those looking to specialize in maxillofacial surgery have a chance to hone their skills in this area.

Each dentist sees a minimum of 21 patients over the five days [1]. Year-on-year, there will be different cohorts, with a new range of dentists partaking each year.

Dentists pay approximately $22,000 in tuition fees to be part of the program [1]. While this is a high cost, it provides invaluable experience which can provide long-term benefits to them.

The benefits

This program is helpful for many people. Dentists are able to specialize in certain areas, which will mean a greater number of dental professionals will be in the Canadian workforce.

Robbins told CTV News Edmonton that “it saves a lot of money”, and that “you get the best of the best” [1]. Continuing, she says that “you are put to sleep, you wake up an hour later, and your mouth doesn’t hurt”, before adding that “you just walk out, and you are done” [1].

As Robbins’ explanation shows, it is a very straightforward system. The patient doesn’t even have to be awake for it – making it very easy for the patient, while giving the dentist plenty of time to finetune their skills.

Comments from the University

Dr. James Yacyshyn is the Continuing Dental Education Director at the School of Dentistry. Yacyshyn commented on the program, saying that “it ends up being a pretty big charitable care initiative” [1].

Yacyshyn says that “patients are saving a lot” – referring to the cost saving element of the plan [1]. Moreover, he also commented that “especially given the cost of care and access to care being the challenge it is these days, it’s nice being a part of this” [1].

While Yacyshyn has admitted that the dentists are making “a huge financial commitment”, he has noted that “it’s a really great program” [1]. Furthermore, he says that the dentists are told that their goal “is to always advance how you are taking care of your patients in practice in your respective community” [1].

Overall, this program can clearly benefit many people, and will hopefully continue to do so. With the success of this program apparent, many other Universities may adopt similar programs in the future.
